What You’ll Find in This Kit

This Population Health Promotion (PHP) Resource Kit offers a variety of tools to help nurses and interdisciplinary health and social service helpers to work collaboratively with each other and diverse Indigenous Nations and community groups to co-create healthy communities for all where everyone has access to the fundamental resources for health including, for examples:

 

 

  • Safe, secure and affordable housing
  • Affordable childcare and elder care
  • Nourishing food
  • Safe, supportive and inclusive communities where everyone is valued for their uniqueness
  • Transportation
  • Education
  • Secure, livable income
  • Meaningful work
  • Publicly funded health care

In the world of population and public health, these resources are referred to as the social determinants of health and this PHP Resource Kit answers the call from the World Health Organization’s 2008 Commission on the SDOH for health workers to reach beyond the hospital walls to work in collaboration with diverse civil society partners in building movements that co-create healthy communities and health for all.
